(644-A) Basic Background Check
All employees, volunteers, and clergy must complete a background check before service in the Diocese of La Crosse. Background Check Applications must be completely filled in and contain the signature/e-signature of applicant. Minors are required to complete background check but do not need to provide their social security number. Background checks must be renewed every 5 years.
(644-B) SE Training Verification
To be filled in and signed, following completion of the Safe Environment Training Program. Verification of SE training is required every year for all employees, volunteers, and clergy.
(644-C) Confidential Questionnaire
To be filled out by everyone involved in Parish, School, or Diocesan events.
(644-D) Verification / Declination of Protect & Heal Training
To be filled out by families with minors participating in all Catholic schools or parish DRE/CRE classes. While the Diocese of La Crosse offers and encourages the use of the Protect & Heal Safe Environment Training Program, families have the choice to opt out of the Protect & Heal program if they so choose.
(644-F) Code of Conduct for the Diocese of La Crosse
All clergy, candidates for ordination, and anyone who works or volunteers within the Diocese of La Crosse at any of its parishes or schools have the legal, moral, and religious responsibility to perform their duties in a way that educates and assists – and does not harm – the people with whom they work.
CATHOLIC MUTUAL INSURANCE REQUIREMENTS
In addition to Safe Environment forms, the Diocese of La Crosse’s insurance needs are administered by Catholic Mutual Insurance. Catholic Mutual Insurance requires additional forms for employees & volunteers who will be driving for the Diocese of La Crosse. Prior to driving on behalf of the Diocese of La Crosse, all drivers must be in compliance of the Vehicle Safety Policy.
